We look at your visit in our office as an orientation to our practice for both the child and parent. You will see that we are very different from most dental offices. We are a team-oriented practice and work as a group in treating your child.

"Prevention" is the key word in pediatric, dentistry. We want you to know all about the care of your child's mouth during the developing years. We plan to accomplish the following on your first visit - detailed oral hygiene instruction, a complete examination of the oral cavity and any necessary x-rays.

In preparing your child for the first visit, stress this as a positive experience. We love dentistry and it is our hope that your child will look forward to future appointments. The book Going to the Dentist by Fred Rogers is a terrific book to share for your child's anxiety about their first visit to the dentist.

By clicking on the following links you will be able to print the Patient Information Form, Medical & Dental History Form, Notice of Privacy Practices and Consent for Use and Disclosure of Health Information. These forms are to be completed entirely and returned to our office at the time of your visit. We ask that a parent or legal guardian accompany the child at the first visit for medical history and permission reasons.
